Benign Prostatic Hyperplasia (BPH) – Enlarged Prostate

Benign Prostatic Hyperplasia (BPH) – Enlarged Prostate

As men get older, it is common for the prostate gland to enlarge. This non-cancerous condition is called benign prostatic hyperplasia (BPH) or prostate adenoma.

Enlargement of the prostateusually manifests in urinary symptoms including:

  • Weak or prolonged urine stream
  • Dribbling after urination
  • Frequent nighttime urination (nocturia)
  • Incomplete bladder emptying and residual urine
  • Urgency and discomfort when urinating

Early and Advanced Treatment Options for BPH

In the early stages, BPH symptoms can often be managed with medication. If drug therapy fails surgical treatment may compose an alternative option.  At the Urology Department in Gronau, we offer a full range of modern diagnostics and minimally invasive surgical procedures which we tailor to your personal situation.

Transurethral Resection of the Prostate (TURP) – The Gold Standard

TURP represents the worldwide “gold standard” for BPH surgery. The excess of prostate tissue causing urinary problems is removed by using a resectoscope. This resectoscope is placed through the urethra and enables the surgeon to see and peel off prostate tissue by using an electrical loop that both cuts and cauterizes.

  • Applicable for moderate enlargement of the prostate up to 100 ml
  • Hospital stay: approx. 5 days
  • Rapid improvement in urination within a few days
  • Tissue examination to exclude other conditions such as prostate cancer
  • Main side effect: retrograde ejaculation (semen enters the bladder instead of exiting the penis)
  • Very low risk (<0.5%) of lasting incontinence

Holmium Laser Enucleation of the Prostate (HoLEP)

For moderate to severe enlarged prostates (80–150 ml or more), the HoLEP procedure is appropriate. The prostate adenoma is removed in fragments by a minimal invasive technique using a laser.

  • No abdominal incision required
  • Catheter for only a few days
  • Lasting effect and quick recovery similar to TURP
  • Tissue examination  to exclude conditions such as prostate cancer

Robotic-Assisted Prostate Adenoma Enucleation (da Vinci®  Surgery)

For severe enlarged prostates (over 120 ml), our specialists offer a robotic-assisted minimally invasive surgery using the da Vinci® surgical system. Through a few small abdominal incisions, robotic instruments are used to precisely remove the enlarged prostate .

Two main surgical approaches are available.

  • Comparable outcomes to TURP and HoLEP
  • Tissue examination to exclude other conditions such as prostate cancer
